Family Nurse Practitioner - Physician Assistant

Description The Family Nurse Practitioner or Physician Assistant will provide comprehensive outpatient primary care for adult patients, as well as evaluate and manage patients with pulmonary and occasional sleep-related conditions. Primary care responsibilities will include preventive care, routine health maintenance, chronic disease management, acute and same-day visits, medication management, health education, screening, and care coordination. The FNP/PA will also evaluate and manage common pulmonary and sleep conditions in collaboration with the supervising physician and clinical team. The position offers an opportunity for an advanced practice provider who enjoys variety and would like to practice both primary care and specialty medicine in a growing outpatient setting. As one of our founding clinical team members, you will play an important role in shaping patient care, establishing clinical workflows, and helping develop both our primary care and specialty services. We are seeking a motivated provider with a strong foundation in primary care and an interest in pulmonary and sleep medicine. Pulmonary experience is highly desirable but not required , as we are willing to provide training and mentorship.
